Challenge

Following multiple work-related fatalities over a fiveyear period, dss+ was engaged to enhance the company’s safety management system and reduce exposure to Serious Injuries and Fatalities (SIF).

dss+ approach
To address the challenges, dss+ conducted a comprehensive assessment focusing on culture, leadership, and management systems, establishing a baseline for future improvements. Over 15 months, the project included key initiatives:

  • dss+ conducted training and development sessions for over 210 leaders and more than 270 employees, introducing four leadership pillars to guide safety improvements:
    • Operating discipline
    • Infrastructure and mechanical integrity
    • Management of change (personnel)
    • Serious Injury and Fatality (SIF) prevention

During this time, some workstreams were established:

  • Created a Project Management Office (PMO) to oversee progress.
  • Developed an EHS vision and strategy aligned with organisational goals.
  • Identified and implemented quick win improvement opportunities, including proactive high-risk reporting, daily safety observations, and established processes for routine validation of SIF critical controls and risk identification.

This structured, phased approach empowered leadership and provided a clear roadmap to enhance safety performance across all sites.
